The following policies exist to help us in achieving our commitment to providing you, our patient, with the highest quality of dental care.
Our office strives to make sure our patients are seen at their appointed time. Occasionally, a dental emergency may arise that can affect our timeliness. We understand that our patients may also have an unforeseen event that may cause them to be late or miss an appointment. In an effort to be fair to all of our patients, we have implemented the following:
- If you arrive 10 minutes past your appointment time, it may be necessary to reschedule your appointment.
- Appointments cancelled or rescheduled with less than 24 business hours notice or appointments that are failed (no show) may be assessed a fee of $50.00. Exception: Appointment reserved for 1 ½ hours or longer will be charged according to procedure type and can be up to $150.00. Multiple or continued broken appointments may result in dismissal from our practice.
As a courtesy to our patients, we will file your insurance claims for each visit. The fees we charge for services are based on the procedure that is to be done. The fees are the same regardless of whether or not insurance is involved. Your policy may base its allowance on a fixed fee schedule which may or may not coincide with our usual fees. You should be aware that different insurance companies and dental plans vary greatly in the types of available coverage.
An insurance policy is a contract between the insured and the insurance company. The dental practice is NOT a party to that contract. In most cases, we will accept assignment of your insurance benefits, process claims, and assist in the appeal of unpaid claims. It is the insured’s responsibility to keep us informed of any insurance changes.
You need to be aware of the benefits and limitations of your insurance plan. Knowing what your plan covers and what services are excluded will be helpful to you in making your decisions regarding treatment. It is important to understand that we make all our treatment recommendations based solely on our patient’s dental health and needs. The insured is responsible for all costs not paid by their insurance company.
We accept cash, checks, and most major credit cards: Visa, Mastercard, Discover, and American Express. We do not carry account balances. All co-pays are due at the time of service.
For our insured patients, claims are submitted to insurance, any any remaining balance will be billed to the account and payable within 30 days. For extensive treatment, we will request a predetermination from your insurance company prior to treatment to determine your financial responsibility.
Also, for extensive treatment, we offer a variety of payment options upon credit approval through a third party – CareCredit. These arrangements must be made prior to treatment and are done through the business office.
